text-decoration: underline;\">Primary Care <span style=\"color: #276b7d;\">Services<\/span> include:\u00a0<\/span><\/strong><\/h4>\n<ul>\n<li><span style=\"font-size: 12pt;\">Diagnosis, management, and support for patients with both acute and chronic physical and mental health problems.<\/span><\/li>\n<li><span style=\"font-size: 12pt;\">Health promotion and disease prevention are integrated into daily health care services.<\/span><\/li>\n<li><span style=\"font-size: 12pt;\">We are committed to integration with other community service providers to optimize the well-being of our patients and community.<\/span><\/li>\n<li><span style=\"font-size: 12pt;\">We link to specialty-based services in Kingston and Perth.<\/span><\/li>\n<li><span style=\"font-size: 12pt;\">We host Community Partners at the Sharbot Lake Family Health Team including Sounds for Life, Primary Care Asthma Program (PCAP), Addictions Mental Health Services-KFLA (AMHS-KFLA), and Geriatric Psychiatry Outreach.<\/span><\/li>\n<li><span style=\"font-size: 12pt;\">We are an Academic Family Health Team and often have Residents and medical\u00a0students from Queen\u2019s Faculty of\u00a0Medicine, and other universities\u00a0participating in the delivery of\u00a0health care.<\/span><\/li>\n<\/ul>\n<h4><span style=\"color: #276b7d;
