{"id":267,"date":"2015-09-02T15:00:52","date_gmt":"2015-09-02T15:00:52","guid":{"rendered":"https:\/\/sharbotlakefht.com\/?page_id=267"},"modified":"2019-11-04T18:25:39","modified_gmt":"2019-11-04T18:25:39","slug":"asthma-program","status":"publish","type":"page","link":"https:\/\/sharbotlakefht.com\/?page_id=267","title":{"rendered":"Primary Care Asthma Program"},"content":{"rendered":"<p><span style=\"color: #333333;\">Did you know that Asthma is considered to be poorly controlled if you say \u201cyes\u201d to any of the following questions?<\/span><\/p>\n<ul>\n<li><span style=\"color: #333333;\">Do you ever have to stop doing physical activity because of your breathing\/asthma?<\/span><\/li>\n<li><span style=\"color: #333333;\">Do you have any of these asthma symptoms (cough, chest tightness, wheeze or shortness of breath) 4 or more times a week?<\/span><\/li>\n<li><span style=\"color: #333333;\">Have you taken your reliever\/rescue medication (i.e. \u201cblue puffer\u201d, Ventolin\u00ae, Bricanyl\u00ae etc.) 4 or more times this week?<\/span><\/li>\n<li><span style=\"color: #333333;\">Do your asthma symptoms wake you up at night?<\/span><\/li>\n<li><span style=\"color: #333333;\">Have you had to go to the emergency room or walk-in clinic in the last 3 months because of asthma symptoms?<\/span><\/li>\n<\/ul>\n<p><span style=\"color: #333333;\">Your doctor or nurse practitioner\u00a0can refer you for Asthma Education.<\/span><\/p>\n<p><span style=\"color: #333333;\"><strong>Everyone<\/strong> who has asthma, or suspected asthma, may benefit from this program.<\/span><\/p>\n<p><span style=\"color: #333333;\">The program consists of one-on-one education sessions (with you and the respiratory educator \u2013Jessica) and possibly a simple breathing test.<\/span><\/p>\n<p><span style=\"color: #333333;\">Ideally, this education is tailored specifically to your needs and can be broken down into 2 to 3 sessions about a month apart followed by annual or semi-annual follow up.<\/span><\/p>\n<p><span style=\"color: #333333;\">At each session you can expect the following:<\/span><\/p>\n<p><span style=\"color: #333333;\">Session #1 (approximately 1 and \u00bd hrs) topics will include:<\/span><\/p>\n<ul>\n<li><span style=\"color: #333333;\">Do you have asthma? What is asthma and how is it diagnosed?<\/span><\/li>\n<li><span style=\"color: #333333;\">Is your asthma well controlled? (This may involve performing a simple breathing test).<\/span><\/li>\n<li><span style=\"color: #333333;\">Determining your asthma triggers and how to avoid\/limit them.<\/span><\/li>\n<li><span style=\"color: #333333;\">What do asthma medications do, how do they work, when to take them?<\/span><\/li>\n<li><span style=\"color: #333333;\">How do I properly take my medications? (<em>Did you know: approximately 50% of people\u00a0w<\/em><em>ith asthma take their medications incorrectly?<\/em>)<\/span><\/li>\n<\/ul>\n<p><span style=\"color: #333333;\">Session #2 and Session #3 (these follow-up visits are approximately 45 minutes long) topics will include:<\/span><\/p>\n<ul>\n<li><span style=\"color: #333333;\">Is your asthma well controlled?<\/span><\/li>\n<li><span style=\"color: #333333;\">Repeat breathing tests to monitor for possible improvements.<\/span><\/li>\n<li><span style=\"color: #333333;\">What do I do if my asthma flares up? How to use an \u201cAsthma Action Plan\u201d.<\/span><\/li>\n<li><span style=\"color: #333333;\">What is a Peak Flow Meter and how do I use one?<\/span><\/li>\n<li><span style=\"color: #333333;\">Review how to properly take medications.<\/span><\/li>\n<\/ul>\n<p><span style=\"color: #333333;\">Annual or Semi-Annual Follow-up Sessions will include:<\/span><\/p>\n<ul>\n<li><span style=\"color: #333333;\">Is your asthma well controlled?<\/span><\/li>\n<li><span style=\"color: #333333;\">Repeat breathing tests<\/span><\/li>\n<li><span style=\"color: #333333;\">Review medications and how to properly take them<\/span><\/li>\n<\/ul>\n<p><span style=\"color: #333333;\">The Primary Care Asthma Program (PCAP) is a ministry funded program aimed at improving patient\u2019s asthma control, self-management and knowledge thereby reducing exacerbation and asthma related deaths. PCAP is delivered at your site by Jessica Schooley (the PCAP Kingston site coordinator). The patient education program consists of one-on-one education and asthma monitoring sessions with the patient and the asthma educator (Jessica). Ideally, this education is broken down into 2 to 3 sessions about a month apart followed by annual or semi-annual follow up.<\/span><\/p>\n<p><span style=\"color: #333333;\"><em>At each session the following will addressed<\/em>:<\/span><\/p>\n<p><span style=\"color: #333333;\"><strong>Session #1<\/strong> (approximately 1 and \u00bd hrs):<\/span><\/p>\n<p><span style=\"color: #333333;\">Basic asthma pathophysiology, asthma control, how asthma is diagnosed, asthma triggers and trigger reduction strategies, medication actions and side effects and assessment of device technique.<\/span><\/p>\n<p><span style=\"color: #333333;\"><strong>Session #2 and Session #3 <\/strong>(approximately 45 minutes long):<\/span><\/p>\n<p><span style=\"color: #333333;\">Assessment of asthma control including spirometry, development of a personalized Asthma Action Plan, peak flow technique and monitoring as well as addressing any potential barriers to adherence.<\/span><\/p>\n<p><span style=\"color: #333333;\"><strong>Annual or Semi-Annual Follow-up Sessions<\/strong> (approximately 45 minutes long):<\/span><\/p>\n<p><span style=\"color: #333333;\">Assessment of asthma control including spirometry, review device technique and action plan as well as any potential barriers to adherence.<\/span><\/p>\n<h2><span style=\"font-size: 14pt;\">For more information about our programs and services please call us at 613-279-2100<\/span><br \/>\n<span style=\"font-size: 14pt;\">or email us at programs.slfht@gmail.com<\/span><\/h2>\n","protected":false},"excerpt":{"rendered":"<p>Did you know that Asthma is considered to be poorly controlled if you say \u201cyes\u201d to any of the following questions?
